Serve youth/families that are experiencing emotional, behavioral, substance use disorders, intellectual and/or developmental disabilities and chronic medical health challenges. There is no charge for CCCC's services.

Provide care coordination and wraparound care planning for children and their families with the most complex needs and are responsible for facilitating access to a full range of treatment and support service.

Facilitate, coordinate and work within child-family teams to develop individualized service plans (ISP) and connect to community sustainable resources.

Keep children in their own homes, schools and in their community. We provide direct, face-to-face care coordination and wraparound care planning for children and their families with the most complex needs (mental health, behavioral, developmental delays, substance use disorder, and chronic medical challenges.

Responsible for facilitating access to a full range of treatment and support services. Linkage to the community.
